Streszczenie
Celem pracy jest przybliżenie lekarzom innych specjalności niż psychiatria zaburzenia lękowego z napadami lęku (PD). Chociaż choroba ta rzadko występuje w populacji ogólnej (1–2%), to staje się ważnym problemem w izbie przyjęć na ostrym dyżurze internistycznym. Badania sugerują częstość występowania PD od 25 do 60% wśród pacjentów z bólami w klatce piersiowej pochodzenia niesercowego. U około 25% pacjentów zgłaszających się na ostry dyżur kardiologiczny z powodu bólu w klatce piersiowej stwierdza się, że jest on objawem lęku napadowego. Lęk rzadko uwzględnia się w diagnostyce różnicowej. Wczesne rozpoznanie zaburzenia lękowego z napadami lęku może bardzo zmniejszyć koszty leczenia przez ograniczenie badań dodatkowych, chroniąc jednocześnie pacjentów przed niepełnosprawnością. Autorzy przedstawiają kryteria diagnostyczne, występowanie zaburzeń lękowych oraz koncepcje patogenetyczne. W końcowej części artykułu omawiają leczenie oraz diagnostykę różnicową zaburzenia.
Abstract
The aim of this paper is to present panic disorder (PD) for medical specialists – non−psychiatrists. Despite rather low prevalence of this disorder in general population (1–2%) it becomes an important problem in emergency service offered by internist clinic. Researchers suggest that about 25–60% of patients with chest pain of non−cardiac origin suffer from panic disorder. About 25% of patients who are admitted at the cardiac emergency unit because of chest pain are physically healthy but suffer from panic attack. Unfortunately anxiety is hardly ever taken into consideration in differential diagnostic by physicians. Early diagnosis of anxiety disorder with panic attacks may significantly reduce treatment costs by limiting the number of accessory investigations as well protect patients from becoming an invalid. Authors present diagnostic criteria, prevalence and patogenetical hypothesis of PD. In the final part of the paper the authors discuss treatment and differential diagnostic of panic disorder.
